How does comment moderation work? #101

In digress.it v2, if a comment was moderated, there was no message to tell the 
commenter this was the case. Has v3 improved on this? Does it alert the 
commenter that their message is being held for moderation? I think that's 
pretty important. I've seen a number of cases where readers are frustrated that 
their comments, sometimes posted two or three times, are not showing up.

I'm glad that Joss raised this issue. Perhaps one solution would be to insert a 
simple 1-line reminder above the submit button, something like "Comments appear 
after approval by moderator." I've seen something like this elsewhere. If you 
want digress.it to be flexible for others, then perhaps make this a 
user-defined phrase in the options settings.

the comment now appears and will say "This comment is awaiting moderation"

this might be buggy. testing would be good.

Comment moderation doesn't work with r204. When the comment settings are set to 
always moderate comments, they are not respected and comments can be made 
unmoderated by anyone.

as of r209 if "always moderate" is enabled, comments will be held in moderation.

it's an on/off switch now... variations of some comments being held due to a 
wide range of conditions is not yet supported.
