Provide information on paragraph output switches in the 'tool tip'

Although we have XML/RSS/JSON/TXT/HTML switches for each paragraph, they
are undocumented and impossible to use because the post id number must be
known.

For example: 

http://test.jiscpress.org/?p=15&digressit-embed=1&format=xml

The switches need to be included as information in the paragaraph 'tool
tip' along with the permalink and embed information. The URIs should be
listed as follows and preferably, be easy to copy and paste.
