Comment browser is incomplete #68

When I go to Comments by User and click on a user name i.e. 
http://b3ta.jiscpress.org/comments-by-user/1/ I can see the list of comments 
but there is no way to navigate to the paragraph where they were made. This is 
a regression from v.2.3.2

Also, only logged in users are showing in the Comment Browser.Is the URL 
correct?

This page, for example, should show 'Joss Winn' and 'admin' but only shows 
'admin'

If I go to http://b3ta.jiscpress.org/comments-by-user/ I get a 404

Also, the General Comments page does not display any comments. This page:

should show this general comment:

Just to add to the first point made above. We need a way to display this type 
of URL:

so that individual comments clearly have a URL.

Should we change 'There are x comments in this book' to 'There are x comments 
in this document' - it's more generic.

Non logged in user's comments are still not showing in the comment browser, but 
I guess you're still working on that.

The comment field is still showing at the top of the discussion thread rather 
than the bottom. See screenshot. Comments are being inserted at the top, too.

There's still something weird going on with the links from the comment browser 
to the comment box. Sometimes, in Chrome, when I click on 'Go to thread', it 
takes me to the post but the comment box is not there or it's aligned too 
vertically and the page seems split in too. Try making a new comment and then 
try it. It seems to go away after one or two attempts.

On this page, there are five paragraphs but only options to comment on four of 
them. When I click the fifth para, well, see screenshot or try for yourself.
