Provide an option to keep the sidebar open at all times/navigation #71

I quite like the sidebar but not everyone will like it. I wonder whether we 
need a way to make it more obvious. Also, compared to v2.3.2, navigation 
between pages is now harder/slower. I miss the forward/back links at the top 
and bottom of each doc section. Can we have them back?

In my view, easy navigation is the most important feature, especially for 
newcomers to digressit. One advantage to v2.3 was that the "Table of Contents" 
button always appeared at top, which many of us rely on to get back the surface 
after swimming deep inside posts and comments. Restoring a TOC button in v3 and 
linking it to the navigation sidebar might be one solution.
