What steps will reproduce the problem?
1. Remove all shipping rates via admin
2. Add item to cart, checkout.
What is the expected output? What do you see instead?

You get a blank shipping selection page, which looks screwed up.
Perhaps in this case we should eliminate the shipping step all together and
redirect to the confirm or complete page?
If doing so, we'd need to modify the view templates to get rid of the "Step
1 of 3" business the checkout page shows.
