Bootstrap polling should respect etags and if-modified-since #18

Otherwise we will get blocked by certain publishers. This work will also
give us the basis for collecting polling stats and adjusting the repoll
frequency.

Applied for all feed fetching, not just boot-strap polling. This should keep the
publishers out there happy with our pulls.
