Poor performance for search Practitioner::mailbox-mss and PractitionerRole::given

Searching Practitioners by mss e-mail is really slow (seconds) compared to the search of RactitionerRole's mss e-mail (<1s). Searching PractitionerRoles by given is really slow (seconds) compared to the search of RactitionerRole's family (<1s).
