unable to install mollify, results into a blank page

What steps will reproduce the problem?
1.After setting up db. and add the config file, clicking continue will result 
into a blank page. I doesn't even make it to the DB confirm page.

What is the expected output? What do you see instead?

What version of the product are you using? On what operating system?
2.5.12 on Ubuntu 12.04 LTS

Please provide any additional information below.
Have already tried the check.php returns OK.
