After clicking Compare & pull request button after making changes on my branch on the forked repository, I see the following screen.

It's not templated, it is just whole content of pull_request_template.yaml.

Perhaps this is not working as expected.

@AlexSCorey Just to inform, I'm tagging you since you've added that file.
