Actual: Screen Reader is not reading the label for the “Proxy Name” and “Description” Edit box.

Expected: Screen Reader should read the label associated with the “Proxy Name” and “Description” Edit box.

User Impact: User with disabilities find it difficult to understand what is the purpose of edit box.
