For results page, link to a short survey (hosted by surveymonkey maybe?)

It is easy to embed surveys by Wufoo. What should this look like? A form at the bottom of the results page, or a link to a popup page? Is it reasonable to use Wufoo, or should we make our own forms and save that data to the wp database?
