Add grammars v4/ Issue 11 #35

This PR contains changes for reading the github grammars-v4 repo, fixing https://github.com/antlr/antlr4-lab/issues/11 at least preliminarily. The changes include:

updates to .gitignore to ignore several temporary directories.
updates to index.html to add the drop-down select controls for grammar and input.
updates to antlr-client.js to read the grammars.json file in the repository (online) and set up the controls.

Note, I could not get axios to work, which you suggested.

Let me pull in manually with the command line and try it out. I might have some clean up stuff I want to do then I can manually merge into master. Thanks a million @kaby76 !!
