"Upload Image" and "Edit Profile" are broken #366

What are the steps to trigger the problem?
1) Try and upload a picture or edit your profile using any up to date version 
of Dabr (such as at dabr.eu)

What happened?
You get a 404 page.

What did you expect to happen?
The changes to my profile to take effect.

Why is this a problem?
When the page titles and URLs for these two pages were updated in r496, the 
action URLs in the actual forms were not updated with them. This is a simple 
fix.
