Please configure log4jconfig.xml to "TRACE" level, and post the log
(catalina.out if using tomcat) contents of the error here.

I must have been doing something weird, because after I changed the logging
level i have been unable to re-create the issue. Maybe restarting tomcat fixed
the issue.
For accounts with proper rights to the pwmResponseSet attribute, the changes
are saved properly. For those that don't have this ACL set we get an "unknown
error" which is expected when it cannot right back via LDAP to the above
mentioned attrib.

I too am experiencing this issue, exactly as described above. I am attaching
the requested log file.
In the log at 13:02:03 was a successful setting of responses. Then at 13:02:46
a second attempt was made and failed.
After a restart of the tomcat5 server, the first attempt at setting Responses
is successful. Any user can do it. After that, anyone trying to set their
responses gets the "Unable to communicate with server. Continue when ready."
message and when submitting the responses they are directed to login again.
I hope this all makes sense, and please tell me if I can provide any further
information on this issue.
