Sometimes switch status stays "on" #3

I have noticed that time-to-time switch status in the app either stays in the "on" state when the device is turned off.

I had an automation that closes blinds in the evening and light turned on, but with the current DH it triggers randomly.

Please let me know what additional information is required to debug this issue
