Cleanup before 5.0.0 #404

Largely cosmetic changes, but potentially API breaking

eliminated some global (to namespace) typedefs
made private some methods for internal use
removed deprecated methods
moved convenience typedefs to the front
moved constants to separate namespaces
some documentation improvements
