Modernize the project to current standards

This PR updates the project to be consistent with our current standards.

We don't expose Cakefile to users and they use npm test and similar commands.
Moved sources in src/
Moved tests into test/
No longer use ancient Node 0.10 for testing
Configured mocha to run directly.
