starknet: update to RPC v0.6

Summary RPC v0.6 is a big change, with new transaction types (V3) among other things. This commit tries to map new fields (like multi-currency fees) to the old ones.

starknet: include transaction index in transaction

Summary Transaction index can be used to generate a unique id or to order events.
