chore: add meta docs #5

This PR copies various .md files from appium/appium. The issue template and PR template have been updated. The contribution guide has been updated to use absolute links back to appium/appium, and adds a note about this.

CODE_OF_CONDUCT.md has been updated to remove mention of Isaac Murchie, and I updated Jonathan's email address via wild-ass guess. I am pretty sure GitHub does not understand CONDUCT.md but does know that CODE_OF_CONDUCT.md is a code of conduct.

GOVERNANCE.md needs updating.
