We'll need some specific info (dmesg, /proc/bus/usb/devices) on the affected systems. The device files should only be shown for ports without a connected printer...

per requests from Till at http://bugs.mandrakelinux.com/query.php?bug=56
IBM ThinkPad A20m 700MHz/512MB/40GB running Mandrake 9.2+ with USB devices including internal (unused) USB, SIIG PCMCIA/USB 4-port, external 4-port hub, HP5550 printer, 15GB HD, mouse, and sometimes Sony camera and/or Kodak 6-in-1 card reader... HTH
Note... I recently created the usb5550 printer and it works although with a different URI that I hadn't noticed -- it was created in an attempt to resolve an excessive halftoning problem when printing pictures (not yet resolved).
My workaround for this bug: ln -s /dev/usb/lp0 /dev/usblp0

OK, so from what I can see, at some point you had connected the printer after CUPS had already started and selected USB printer port 1 via the device file instead of the full URI. The device filename reported by the USB backend was incorrect (a copy of "ls -l /dev/usb*" would be useful) and so you weren't able to print until the next time you configured the device URI with the make/model info.
Can you post a copy of "ls -l /dev/usb*" so that we can track down the core issue? Thanks!

/dev/usblp0 is hardcoded.... at the time I was looking at this, I did a "strings

/dev/usblp0 is one possible device filename out of three that the backend looks for. Basically, if /dev/usb/lp0 and /dev/usb/usblp0 do not exist, it falls back on /dev/usblp0.
Are you using devfs? That might explain things, as when the USB printer is not connected the /dev/usb/lp0 device file will not exist...

devfs: yes... though /dev/usblp0 didn't exist either, so I gotta wonder why it was chosen...?

Just added a new USB Epson Stylus CX6400 and it too would not work until I issued "ln -s /dev/usb/lp1 /dev/usblp1"...

Unfortunately, we don't have any systems configured with devfs at the moment. AFAIK it is still considered experimental...
My guess would be that access() doesn't work with devfs devices, which is causing the problem.

Since devfs is deprecated in Linux 2.6 and as there is no apparent fix we can provide, I'm closing this out.

In some distros USB printers appear as /dev/usb/lp0 on others as /dev/usblp0. Recently appeared the problem that on a distro with /dev/usb/lp0 CUPS tried to access the printer under /dev/usblp0. There are several reports about this problem:
