Version 3 PR:

Moving to Node 4, deprecating Node < 4.x.x.
Removing options for retryOnDisconnect as this feature is now stable (fixes numerous bugs).
Preliminary support for Windows (thanks @laggingreflex).
Solidified exclude functionality (thanks @laggingreflex).
Numerous bugs, which I'll close once it's in 3.0.0 but:
- No more double-messages on reconnect.
- Deleting files doesn't result in closing the remote host.
- Excludes are now properly excluded.

I'm going to merge this PR and get a 3.0.0 build out on npm. Will address bugs as the surface!
