Combine Dockerfiles
Add ENV variable support for: token, log level, plugin choices
Default to kube-system namespace
Lock appoptics-snaptel dpkg version
Combine ServiceAccount and Deployment yaml files
Simplify installation

Resolves Issues https://github.com/appoptics/kube-ao/issues/10 and https://github.com/appoptics/kube-ao/issues/13.

Would it make sense as a next step to go and create a HELM Chart under https://github.com/kubernetes/charts ? Will make it easier to install and manage in general
