Are you using version 1.25 of Kindle for PC? If so, uncheck auto-update and go back to at least version 1.24.

calibre Debug log

How did you produce a calibre debug log? I can't find any options in Calibre to do so. I used "restart in debug mode", but this has no visible effect.
