Replace the `@zestia/ember-auto-focus` addon with a simple custom modifier #468

The addon is no longer published to npm and we were only using it in one place, which could easily be replaced by a simple modifier.

This might affect apps that were using the addon as a shadow dependency. They would have to install the addon explicitly now.
