What's the preferred way to extend a decklist beyond the available slots? Extra copies of the sheet with the other cards filled in there? (should the name/DCI#/other metadata fields be filled out for those other sheets if so?) Should they be listed on a blank page?

I'll defer to you on the format, since I don't have any judging experience to rely on here.

I think I would just say "Main Deck Continued" with the two main rows taking up the entire sheet, same width as the existing ones. SCG I think should be the same, just extend their three columns down onto an additional sheet.

I don't believe there is any "preferred" way to do it, since it doesn't happen too often. I think it's likely to pop up though, especially if Brawl becomes a competitive format.
