New Sample Type Definition Form Hamburger menu > Sample Type Definitions > New (button far right)

Current v2 experience:

• Initial load shows add fields button and field titles without form inputs
• Form uses default inputs, save with default names
• Fields form type is set to string by default

v3 implementation:

• Only show add button on initial render
• Use placeholders instead of defaults
• Do not set field form type

Design decisions per conversation with @spcoon 11/30

• Each required input label should have a red asterisk
• The should be a single asterisk definition per form (right side)
• Top two fields should be "Name" and "Description"
• Remove "Fields Label"
• Field name label should be "Field Name", the button will be below field inputs
• A single empty field will be rendered on initial form load, a user can delete this input
• Remove defaults and placeholders from inputs
• Remove "?" and boolean checkbox labels
• Submit button will be disabled until all required fields are filled, no helper texts
