Is it possible to support link to h1/h2/h3 section titles in .md files? #23

Thanks for the great plugin!

However, I've been playing around with it and I'm curious if it's possible to not only be able to link to notes, but also the titles inside .md files? I think that'd be great in terms of organising thoughts!
