Adding issue and PR tempaltes for bug reports and feature requests. #63

With more folks adding bug reports and feature requests, we need to make it easier to submit these types of issues to the repo. This PR creates these templates for Issues and PRs. The templates also assign the relevant labels.

I've created a version of this PR where all commits are signed in #64, but I can't merge it without approval. I'm closing this in favor of that version for now.
