I don't know the answer to that (I don't use Windows). I assume you've searched
Google, e.g. http://ivascucristian.com/how-do-you-create-an-apache-module/
Presumably somewhere there is somebody taking normal Apache modules and
building them for Windows (using Cygwin perhaps?).
Since this module is not really "special" in any way, whatever resource or
procedure people are using to build other 3rd party Apache modules ought to
work for mod-authn-otp as well.
Let me know if you find it somewhere so I can I'll update the web site... just
add a comment to this issue.

I am searching for an working windows apache module of mod-authn-otp too, so if
someone was successful with that I really would be happy for an short note
how-to.
