add max valueset extension to SD for Required codeable concepts #21

max valueset extension: http://build.fhir.org/extension-elementdefinition-maxvalueset.html will be added to spreadsheet by GG once in will update the bindings for Required codeable concepts and update the guidance in the guidance page on how to how to handle this when all you have is text. and chage summary binding to required or text binding with link.

tooling not supporting invariants on complex extensions may have to do directly in xml. check with GG first. for now making the text element required for
