Commenting should all take place within a single page

To reproduce:

1. Find a story
2. Post a comment

You will end up on a page with just the comment and edit/back links on it.

Instead, comment-posting should all happen within the page for the topmost object being commented-on. Posting a comment should make it appear within that page.
