the fact that it works for some items and not others means the other items are
either set to a system category or are being collected by a rule with a lower
order number
go into edit mode and check where its assigned.

I have the PvP set as the lowest order number (1) to make sure its not that. I
deleted all the other rules as well, to make doubly sure. Ill look into the
'system category' thing, but I cleared that bar out. I assumed rules I actively
add would have higher priority, is this wrong?
Found that as you said some gear was oddly in some system category. Reset the
item and it works. My fault, bit confusing.

Downloaded "pure" AI, wiped all settings and tried to create rule from example
: type( "armor", "weapon" ) - it doesnt apply! But rule name( "something" )
applyed well.
That issue appeared very long time ago, but now im realy need to soft items in
the way i want.

I am having similar issues and I have installed the latest version of AI
through the curse client.
Cata gear will not sort into my "xxx gear" bar and I am using rule like so:
equip() and type("armor") and subtype("cloth") and not sb()
Pre-Cata gear sorts properly for the most part, except things like Green Winter
Clothes and things like that.
Certain weapons will not sort into my weapon rule either.
I have looked at the debug info and gear appears to be marked as Type:Armor and
Subtype:xxx (cloth, leather, mail, plate).

Everything is working fine for me, and I use a whole lot of very complex rules, and also this issue is ancient.
I recommend it be closed.
