[IG modification] report naming modification to cohort code #104

Problem

The extension name of document full text has changed in the ig. DocumentReference.document-reference-raw-text has been renamed to DocumentReference.full-text.

Description

The cohort360 part concerned by this modification is the cohort definition with the document search.
