email2fax only sends cover page #888

What steps will reproduce the problem?
1.send email to email2fax addr with a pdf attachment
2.check fax sent
3.check fax received

What is the expected output? What do you see instead?
On fax sent and fax received you will see the cover page only that is produced 
from the body of the email. the attachment does not get sent.

What version of the product are you using? On what operating system?
Debian 7 master/master BDR

Had Ian (iaokes) test on his system and got the same result.

Can't reproduce the issue.  Just sent a cover page and PDF attachment 
successfully via email2fax feature (see attached file).

Please double check the server for dependencies: ghostscript, tiffinfo, 
tiff2pdf, tiffcp are all required.  libreoffice (headless) is only necessary if 
working with files other than PDF and TIF.

Please also check folder permissions.  I just added some additional 
folder/permission checks to fax_send.php (in r8046).  Probably isn't the cause 
of the problem, but have seen stranger things.

Perhaps you could post the offending PDF for additional testing (?).

Please advise.

I have checked those packages and they are all up to date. i will get more info 
in the next 24 hours and post here.
