Extensions not added #910

What steps will reproduce the problem?
1. Adding one or more extensions isn't working

What is the expected output? What do you see instead?
- An extension should be added and visible. 
- Message 'Add completed' appears, but extension is not added. (not visible in 
account > extensions and also not added in database)

What version of the product are you using? On what operating system?
Linux Debian 3.2.65-1+deb7u2 x86_64
FusionPBX: 3.7.1
Revision 8208

Please provide any additional information below.
Was working before the upgrade
