Table of contents #4

There's a lot of content in this readme and I got lost in it easily. This PR adds a table on contents to easily browse the topics.

The TOC should be generated using tools such as https://github.com/ekalinin/github-markdown-toc.go So, it should follow the format generated by such tool, and then there would be no need for editing manually.

@kkweon Great idea! I used a generator to create the table of contents.
