Add insert.bed for each scheme #4

This PR adds the *.insert.bed files which are created by the latest version of primal scheme. It also updates the README to give some more information on the available schemes.

I've also changed the license from MIT -> CC-BY-4 as I didn't realised it was already licensed in the SOP. Now the scheme license in this repo matches.
