Undefined indexes #2

What steps will reproduce the problem?
1. Run the test script

What is the expected output? What do you see instead?
Clean output of tables.  I see the tables along with lots of Notice statements 
from PHP

What version of the product are you using? On what operating system?
Latest hg clone, Ubuntu 10.04. PHP Version 5.3.2-1ubuntu4.5

Please provide any additional information below.

For me, at least, the test file generates multiple instances of each of these:
Warning: Illegal offset type in Pivot.php on line 108
Warning: Illegal offset type in Pivot.php on line 109
Warning: Illegal offset type in Pivot.php on line 121
Warning: Illegal offset type in Pivot.php on line 122

This is a brilliant script, but waaay over my pay-grade! Definitely worth 
fixing (or showing how to make PHP configurations compatible).
