rename unicode.h to a cconv specified name

currently you install unicode.h to /usr/include/, but it will pollue the 
namespace for other software, how about rename it to cconv_unicode.h?

Not only the header file name, but all the exported functions utf8_char_*() as 
well. I would suggest renaming them with cconv_ prefix.
