Fixes: #77, #78, & #79.

Other changes:

Splits up add.php into different segments in the modules folder under add_comic dir for ease of accessing/maintaining.
Adding custStoryName and quantity fields to the formprocess, editing, and display. Quantity display hasn't been integrating into single comic yet.
Fixed bug in edit mode where if user bought when released, the radio button wouldn't fill in.
Fixing custPlot for single_comic pages
