removed some ifdefs from readme #69

as a potential solution to #68 - very simple, just removed some ifdefs.

You may have a look at my fork https://github.com/gernotstarke/asciidoclet to inspect the results...

According to PR #66 README.adoc is also used on https://asciidoctor.org. That PR may is probably what broke the README. (See @mojavelinux 's comment on Issue #68)

We should probably close this PR and wait for a fix from @mojavelinux that works in both places.

I don't think this PR is necessary anymore. It wasn't the ifdefs that are the problem. Those are necessary for the README to be used in multiple places. The problem was that the header was broken due to a typo (a semi-colon instead of a colon).
