DEFAULT_TOCLEVELS seems to be gone in recent Asciidoctor implementations #130

Using the following line results in an error during my run: https://github.com/asciidoctor/asciidoctor-backends/blob/49e1cac496d187f08f881759a7c182568e3aae0f/slim/html5/block_outline.html.slim#L2

The runtime complains that DEFAULT_TOCLEVELS is not defined

toclevels ||= (document.attr 'toclevels', ::Slim::Helpers::DEFAULT_TOCLEVELS).to_i

This repository is no longer being maintained. If you'd like to submit a PR to fix it, I will merge it. But otherwise, I won't be dealing with this issue.
