An Extension for Defining Additional Replacements #102

An AsciiDoctor extension that provides certain additional replacements (of the sort that would have been set in the [Replacements] section of asciidoc.conf when using AsciiDoc Python). This first cut includes (o) for the degree symbol, 1/2, 1/4, 3/4, and a dozen fairly common words that contain acutes, umlauts, and/or cedillas (entree, cliche, soupcon, naive, etc.)

BTW, Despite being known as polyglot-jones, my Ruby is weak. So, if there's a better way to write this code, I'm all ears.
