monkey patch lib/asciidoctor/reader.rb#PreprocessorReader.process_line to only process unescaped preprocessor directive "include::"
add option to make the feature optional
add include:: directive as comment (see #124)

Drawback: if the process_line method is changed for the include:: directive, then the script should be adapted too.

I'm not accepting changes to this script at this time because I'm currently in the process of graduating it to its own project with a new code base that is far more accurate. See #126.
