A. Are you setting it in the right place? If it is a 'header attribute' (<<>>) it must be defined in the document header (or on the command line), otherwise it won’t work. Also, the document header ends at the first blank line or the document title, whichever comes first. So if you have a blank line between lines in the header, the attributes after it are being defined in the document, not the header. (<<>>).
