Preview does jump down when the document is saved (sync is deactivated) #272

Description

I have deactivated the sync, because it always has a major offset. I have set the option "Render on Save only" I did this to keep the rendered preview nice and steady so I wouldn't loos track about may position in a bigger document. Unfortunately the preview does jump always considerably on save. Its always down. I couldn't figzure out a rule applicable (next headline? next rendering problem?, ...)

Please help to remove this as it make the preview really hard to use.

Line numbering in Asciidoctor 1.5.6 could be wrong in lists and tables.

In Asciidoctor 1.5.7, things had improved:

Good log messages demand good line number tracking. In this release, the line numbers reporting is much more accurate, especially for lists and tables. When reported, line numbers will be within a single line of the source of the problem.

In theory once we upgrade to Asciidoctor.js 1.5.7 it should be better. Having said that I don't see lists or tables in your example so it may be another issue.

Could you please share the whole document in plain text so we can reproduce this issue ?
