Upgrade to Asciidoctor.js 2.0.3

I'd really like to see this upgrade happen soon. Most of the support tickets that come in seem to be the result of using an outdated version of Asciidoctor. It's really important to stay current so we aren't giving users different experiences from using different tools. We want to stay as consistent as possible, even if it means discovering new issues in the process.
