Allow option to always display additional info fields

This is not a defect, rather a compile-time (via setup options to 
CR_INSTALL_INFO) (I guess?) enhancement. Or it could be done via compile-time 
option to CrashRpt itself (#define or similar).

What is the expected output?
It would be nice to have an option to always show the 'Provide additional 
details' fields.
We suspect a lot of our users are unaware of the functionality, and then don't 
enter these values.
Note that this does not make the entering of these values compulsory, it is 
simply a discovery issue.

Ok. I would prefer to implement this through a new CR_INSTALL_INFO::dwFlags 
flag.

We have a big need for this as well, most customers just don't seem to notice 
the link to add additional info, and so very very few leave it.
