When a nut + washer is threaded on to the torque-plate on the opposite side of the arm, it causes binding against the bearing and the nut tends to become loose.
It seems like the mechanism functions OK without any nut. Maybe just eliminate the nut from the assembly?
Leave out the nut for now.
