maybe a nested for_each example, where the outer for_each is tbb, inner is cuda

i'm not sure this is necessary given the docs on the wiki and i can't think of 
an example which isn't lame, like what i suggested above. any addition would 
complicate our build and i'm not sure its worth it for such a lousy example

if we think of a good example, we can add it
